How the Application works and Deadlines for filing:
In order to be eligible for consideration, all documents requested must be submitted at one time. Incomplete submissions will not be considered.
All submissions must be received no later than the following dates:
Fourth Quarter 2008: July 15th, 2008
First Quarter 2009: November 15th, 2008
Second Quarter 2009: February 15th, 2009
Third Quarter 2009: May 15th, 2009
Fourth Quarter 2009: July 15th, 2009
Information, Terms and Conditions
Legal Services for One Free Egg Donation Agreement shall be awarded per fiscal quarter, a value of $1,000.00.
Legal Services for One Free Gestational Carrier Agreement shall be awarded per fiscal quarter, a value of $1,500.00.
Services cannot be combined (we will not award legal services for both an egg donation agreement and a gestational carrier agreement to the same applicant).
Duplicate submissions in one calendar year will not be accepted.
The falsification of any information submitted shall automatically disqualify the application from consideration.
Two “Runners Up” will be awarded to two applicants in each category:
Each runner up will receive a 20% discount on legal services in their designated category of submission for consideration for a free contract.
All applicants will receive a list of resources (including contact information) for ways to finance your cycle.
All information submitted will be reviewed by and considered by the staff of the Law Office of Elizabeth Swire Falker, Esq., P.C., including its agents, employees, representatives and affiliates. We reserve the right to request additional documentation, and to verify all information submitted. No representations or warranties are hereby made or shall be implied as to the outcome of any Application.
We reserve the right to reject any application for failure to comply with the conditions set forth herein.
